The symptoms of ALS appear gradually.
So gradually, in fact, many patients may miss the earliest signs
The first symptoms patients tend to notice include:
- Twitching
- Cramping
- Stiffness in muscles
- Muscle weakness affecting an arm or leg
- Slurred or nasal speech
- Difficulty chewing or swallowing
Muscles damaged early on by the disease are often the parts of the body that show symptoms first. Symptoms may be subtle—patients may notice that they are tripping more often when they walk, or that they are having trouble buttoning their shirt. Overall, muscle weakness is the predominant initial symptom, occurring in about 60% of patients.
As the disease progresses, additional parts of the body become damaged as the muscle weakness and atrophy spreads. For a patient to be diagnosed with ALS, they must present with symptoms in of both upper and lower neuron damage that cannot be attributed to any other condition.
Typically, ALS does not affect cognitive abilities. However, evidence exists that a small percentage of patients may, in fact, experience difficulties with their memory and decision-making abilities, even developing some form of dementia. It's important that patients make informed decisions about their health care in advance, in case there is any diminishment in cognitive abilities.
In the later stages of ALS, the muscles of the respiratory system become increasingly weak, eventually requiring mechanical assistance to help with breathing.
Important Safety Information
- You should not take RILUTEK if you are allergic to any of its ingredients.
- Your doctor should do blood tests to check your liver function before and during your treatment. Call your doctor if you have a fever, cough, or difficulty in breathing while taking Rilutek.
- RILUTEK may cause you to feel dizzy, light-headed, or sleepy. If you experience any of these, you should NOT drive a vehicle or use machinery.
- If you miss or skip a dose of RILUTEK, do not take any extra tablets to make up for those you missed, but take your prescribed dose at the next regularly scheduled time.
- When taking RILUTEK, avoid drinking excessive amounts of alcohol; alcohol may contribute to compromising liver function and may be associated with an increased risk of liver problems.
- The most common side effects of RILUTEK that occurred during medical studies were weakness, nausea, lung function decrease (not associated with a reduction in vital capacity), headache, nasal symptoms, muscle tightness, abdominal pain, and high blood pressure. If any side effects become troublesome, contact your doctor.
- Be sure to tell your doctor and pharmacist about all other medicines you are taking, including nonprescription products and vitamins. If you have questions, please talk to your doctor.

Patients may experience a host of other symptoms, including:
- Dysarthria—difficulty in speaking or forming words
- Dysphagia—trouble moving or swallowing
- Spasticity—upper neuron problems that lead to tight and stiff muscles
- Hyperreflexia—exaggerated reflexes, including an overactive gag reflex
- Fasciculations—lower motor neuron problems that include muscle weakness and atrophy, muscle cramps, and quick twitches of muscles that can be seen under the skin
Indication
RILUTEK (riluzole) is indicated for the treatment of patients with amyotrophic lateral
sclerosis (ALS).
RILUTEK extends survival and/or time to invasive breathing assistance.
